Letter Issued To Graduates that have lost their Original Academic Certificate and to be Signed by the Vice Chancellor & Registrar

Requirements For LETTER IN LIEU OF LOST CERTIFICATE

• Sworn Affidavit and Police Report on Loss of Certificate
• Copy of Notification of Result
• Copy of valid means of identification (i.e. International Passport, National ID Card, Voter's Card or Driver's License)
• Copy of Marriage Certificate or Newspaper Publication on Change of Name (Applicable ONLY to married women who have changed their maiden name)
• White Background Passport size Photograph (jpeg or jpg format only and it should be renamed using full name before upload)
• Relevant Documents except Passport Photograph MUST be in pdf format
