Topic: Global Surgery And Paediatric Urology

Description: My research focus is on Global Surgery and Paediatric Urology. The focus in Global Surgery is to improve access for surgical care in our setting especially for children. My interest in Paediatric Urology is particularly in the management of congenital anomalies of the kidney and urinary tract.Global surgery is the aspect of global health that focuses on mitigating the disparities in surgical access and outcomes between countries with different levels of human development indices. The work of the Lancet Commission on Global Surgery LCoGS showed that five billion people worldwide lack access to timely, safe and affordable surgical care and majority of these in Low- and Middle-Income Countries LMICs especially Sub-Saharan Africa SSA . In SSA, children constitute up to 50% of the population and congenital anomalies constitute up to 9% of surgical burden of disease globally, two-thirds of which are avertable with surgical intervention.I obtained an MSc in Global Health with Global Surgery, and the topic of my thesis was Access Barriers for Children s Surgical Care in Lagos . My research activities with the Global Initiative for Children s Surgery GICS culminated in the development of the Optimum Resources for Children s Surgery document. The recommendations of the LCoGS require development of national surgical plans by member countries of the World Health Assembly. I am a member of the National Surgical, Obstetrics, Anaesthesia and Nursing Plan committee for Nigeria which did a national survey of the status of surgical care in Nigeria, developed the Strategic Priorities for Surgical Care StraPS document for Nigeria, and has been tasked with the implementation of the plan. I am involved in the NSOAP activities of the West African College of Surgeons and recently put in charge of the data and research subcommittee. In the past year, I have been involved in and researching into decolonizing global surgery having been a member of the Transformational Dialogues, a broad coalition of individuals and organizations from the global surgery community working to identify and address issues of neocolonialism and inequities in global surgery.My research focus is highlighted by the research that I have led and the research collaborations that I have been involved in with the resultant publications. These have largely focused on improving access to quality surgical care and improving surgical outcomes especially reduction of surgical site infections and perioperative morbidity and mortality.My current research focus on gastroschisis is aimed at determining the true incidence of gastroschisis in our setting and identify infanticide if present. This will be a premise to provide training at the community level to enhance prenatal screening and prompt referral of the patients for surgical care. This will improve the delayed presentation that often results in poor outcomes in our practice.The ongoing research in paediatric urology is focused on early diagnosis and intervention for upper urinary tract anomalies especially pelvi-ureteric junction obstruction. The other urological research is based on circumcision and aimed at evaluating the training level of the nurses at the primary health care level who perform most circumcisions with a target to reduce high rate of post-circumcision complications.

Qualitative and Quantitative Study of Incidence of Gastroschisis

Research Details

Community based study to assess the occurrence of Gastroschisis.Introduction: Newborns with gastroschisis often present to our centre several hours after birth with inappropriate transfer circumstances which predispose them to sepsis and poor outcomes. The mothers either did not have prenatal ultrasound scan or the ultrasound scan did not diagnose the gastroschisis. Compared with other parts of Nigeria and other parts of the world, the incidence of gastroschisis appears low based on the number of patients presenting to the tertiary hospitals.Methodology: This study will survey traditional birth attendants and nurses at primary health centres to assess their knowledge of and encounter with gastroschisis, their beliefs and attitude to the disorder and practice of management. In addition, there will be focus group interview of community members to determine their knowledge of and attitude to congenital anomalies including gastroschisis.The primary outcome is the estimated incidence of gastroschisis and secondary outcomes are management practices and beliefs about congenital anomalies. The outcomes will inform health education packages about gastroschisis and patient transfer and advocacy for free health care for structural congenital anomalies that require emergency care.
